Treatment support to help your patients get informed and stay on prescribed treatment
Once you have decided INVOKANA® is right for your patient, Janssen CarePath provides additional support to your patients, including:
Patients and caregivers can create an online account where they can learn about their insurance coverage, enroll in the Janssen CarePath Savings Program and manage their benefits, sign up for prescription reminders, and find support throughout their treatment journey.
- Care Coordination
Janssen CarePath provides additional support that your patients may need to get started with their treatment. A personally assigned Janssen CarePath Care Coordinator will work closely with you and your patients to provide the support you direct.
- Janssen CarePath Trial Offer
A free 30-day trial offer is available for eligible patients to help them become familiar with INVOKANA®. At the conclusion of the program, you and your patient decide if it is appropriate to continue treatment. One (1) use is allowed per product, per lifetime. Terms expire at the end of each calendar year and may change. This Trial Offer is open to patients who have commercial insurance, government coverage, or no insurance coverage; however, there is no guarantee of continuous accessibility after the program ends. - Daily medication and prescription refill reminders
Patients can sign up for phone calls, text messages, or e-mails that remind them to take their medication as directed.

Sign Up or Log In to the Provider Portal at JanssenCarePathPortal.com where you can request and review benefits investigations, enroll eligible patients in the Janssen CarePath Savings Program and view their Savings Program transactions.
Patient insurance benefits investigation and other Janssen CarePath program offerings are provided by third-party service providers for Janssen CarePath, under contract with Johnson & Johnson Health Care Systems Inc. on behalf of Janssen Pharmaceuticals, Inc. Janssen CarePath is not available to patients participating in the Patient Assistance Program offered by Johnson & Johnson Patient Assistance Foundation. The availability of information and assistance may vary based on the Janssen medication, geography and other program differences. Janssen CarePath assists healthcare providers (HCPs) in the determination of whether treatment could be covered by the applicable third-party payer based on coverage guidelines provided by the payer, and patient information provided by the healthcare provider under appropriate authorization following the provider’s exclusive determination of medical necessity.
